NC Medicaid is releasing an updated Tailored Care Management Provider Manual to reflect changes to the Tailored CM model since the updates released February 23, 2023. Notable changes include updates to the Acuity Tiers, Acuity-Based Contacts and Contact Monitoring and addition of information on the Healthy Opportunities Pilot Program Launch for AMH+s/CMAs.
