As implementation of the new online system for death certificates, North Carolina Database Application for Vital Events (NC DAVE), continues we want to hear from you!
Under state law, physicians, physician assistants and nurse practitioners may certify deaths. These individuals who sign death certificates are referred to as “medical certifiers”. Any Medical Certifier who signs death certificates or could need to sign one in the future must be registered for NC DAVE.
Medical certifiers who use paper death certificates can be fined as of September 1, 2022. Assessments begin with a penalty of $250 for the first violation, $500 for the second violation, and $1,000 for the third and each subsequent violation.

TNC Vital Records has created an online learning portal for medical certifiers. To register for an account and access training, please follow the steps below:
1. Go to this website.
2. Enter username NCDAVE and password NCDEATHS to login.
3. Click on the green NCDAVE Login button at the right of the page. Select New User Enrollment to register for your account.
I find it ironic that the state wants us to move to electronic death certificates but does not put in an “electronic” way to ask questions, get help, or reassign death certificates. We have to call a phone number that is never answered nor calls returned when messages are left.
when you get your email that tells you there is a death certificate to be completed, usually it goes easily to locate and complete. when you do not, and must locate the individual within the system, there is a variety of difficulties. I was using the system for 6 months before I realized NCDAVE had assigned me two user names with different addresses. Some of the certificates were going to the second address that I did not know existed. A colleague had the same experience.
NC DAVE has made my family’s loss even worse. They never ca or return emails to assist his doctor in getting everything implemented. Meanwhile our loved one remains in refrigeration. This new system is a train wreck to get any response. Dr states they have left multiple voicemail and sent multiple emails with no response. Maybe contacting a lawyer would get someone to respond.
Everything that Chandra said above. This has made a difficult situation for our family much worse. My mom’s PCP was enrolled, but somehow got disenrolled (they didn’t do it). They have gotten little to no help from the state with resolving the issue. Finding one of mom’s doctors who is*actually enrolled* to sign the death certificate is impossible so far. Not an impressive rollout of this system…
I spent over 5 hours during the weekend trying to complete a death certificate of my patient. Left message x 2 on HELP line on Monday with no response. Finally got hold of HELP line and I was told that the death certificate was already completed by another physician!
Better wait to have someone at HELP line to walk you through the entire process, so you do not waste your time.
System is user unfriendly at the very least.
