1. Workforce Self-Care Resource Repository

The Association of Clinicians for the Underserved’s (ACU) STAR2 Center is excited to share a new resource bundle focused on self-care for the health center workforce. This resource repository is meant to serve as a starting point for organizations to grow their support of self-care for their workforce.

 

2. State Designated Rural Health Centers Support Grants

These grant funds, supported through the North Carolina General Assembly, are for assuring access to primary and preventive care to meet the health needs of rural, underserved and medically indigent patients in North Carolina.  An important component of this work is to stabilize and strengthen the safety net through increased levels of collaboration and integration of services to sustain and meet the needs of those served more effectively. The Centers for Medicare & Medicaid Services (CMS) has redesigned the Global and Professional Direct Contracting Model (GPDC) Model to advance Administration priorities, including a commitment to advancing health equity, and in response to stakeholder feedback and participant experience. Assessing Member Eligibility for Participation in the Healthy Opportunities Pilots

Wednesday, March 9, 2022, 1:00 PM Eastern Time

In order to receive Healthy Opportunities Pilot services, members must meet specific qualifying criteria, including:

• Be enrolled in Medicaid managed care
• Live in a Pilot region
• Have at least one qualifying physical/behavioral health criteria
• Have at least one qualifying social risk factor

Care managers will play an essential role in assessing an individual’s eligibility for the Pilots. This session will provide care managers deeper-dive training on the qualifying physical/behavioral criteria and social risk factors that make someone eligible for the Pilots.
