got plansIn April, health care providers across the state will be hosting FREE Advance Care Planning Workshops scheduled to celebrate National Health Care Decisions Day (April 16th). The public is welcome and will receive free information and assistance with completing Health Care Power of Attorney and Living Will forms.
The Health and Elder Law Sections of the North Carolina Bar Association, NC Partnership for Compassionate Care regional coalition members, local attorneys and health care providers across the state are providing information and tools to help those who want assistance in talking about their wishes with family, friends and health care providers. There will also be an opportunity to execute written advance directives (Health Care Power of Attorney and Living Will) in accordance with North Carolina state laws for participants wishing to complete the documents. These resources and documents are available online under the resource tab at www.gotplans123.org. You may also see the list and register for one of the 22 workshops being held throughout the state this month on this site.
These clinic workshops, will cover:

  • Health care powers of attorney
  • Living wills
  • Advance directives*
  • Talking about your wishes for end-of-life treatment

*Please note: You will need to bring a photo ID in order to have your advance directives notarized.
