The NC Population Health Collaborative is comprised of representatives of existing and aspiring NC ACOs, community partners, and patient advocacy organizations from across the state. The Collaborative provides an opportunity to engage in a shared goal of improving population health and health equity in North Carolina through shared experiences and ideas about transitioning to accountable, value-driven, health care delivery systems, and to identify and discuss possible solutions for facilitating that transition. The Collaborative meets twice a year.
